Background
As pharmacotherapy is becoming more complex in recent years, outpatient prescription audit by a community pharmacist is a key in keeping proper use of medications. However, available information on patients in community pharmacy settings is much limited compared to that in hospitals. To date, more hospitals in Japan are issuing..
